Crescent Medical exists to restore time, trust, and thoughtful decision-making to healthcare.

We provide comprehensive primary care and advanced preventive services through an enduring relationship between each patient, Dr. Fatima Jaffrey, and a team that knows their story. Our mission is to combine medical science, careful listening, transparent communication, and a deeply held ethic of service to help people live healthier lives.

By operating outside the volume-driven insurance model for routine primary care, we can focus on what matters most: understanding the whole patient, identifying risks early, coordinating care carefully, and measuring whether treatment is meaningfully improving health and quality of life.

Crescent Medical was founded on a simple conviction: excellent medicine requires time.

Traditional healthcare often divides a person’s health among brief appointments, disconnected specialists, isolated diagnoses, and treatments that are rarely evaluated together. Crescent offers a different experience.

Our limited patient panel allows us to provide longer visits, deeper clinical review, direct communication, and continuity over time. We consider symptoms, medical history, medications, family risk, sleep, nutrition, emotional well-being, hormones, environment, and daily function as connected parts of a person’s health.

Patients are not numbers or collections of diagnoses. They are whole people—with histories, responsibilities, priorities, and wisdom of their own.

Crescent Medical was founded by Dr. Fatima Z. Jaffrey, a family physician with extensive experience in academic medicine, healthcare quality, clinical outcomes, and health-system transformation.

Before founding Crescent Medical, Dr. Jaffrey helped lead quality and performance-improvement work across complex clinical settings within Northwell Health. She also served as an Assistant Professor at the Zucker School of Medicine at Hofstra/Northwell and contributed to the education of future physicians.

Her advanced leadership training through Dartmouth and the Tuck School of Business strengthened a philosophy that continues to shape Crescent today: healthcare should not be judged only by how many services are delivered, but by whether people feel better, function better, and receive the right care at the right time.

At Crescent Medical, that systems-level experience is brought into a highly personal setting—one patient at a time.
